If your facility is performing maintenance on machines, you should have a lockout / tagout (LO/TO) program in place. Part of the requirement of having a properly developed program is to do an annual review of the program. Since many injuries are a result of not having an effective LO/TO program, take a hard look to ensure that you have an effective program that protects your employees.
